What about Guardians of the Galaxy? It has to tie into what they are trying to do with Thanos somehow. So the timeline for the next set of movies appears to be (at the moment):
Iron Man 3 - May 3, 2013
Thor: Dark World - Nov 8, 2013
Captain America: The Winder Soldier - Apr 4, 2014
Avengers 2 - May 1, 2015
Wait a sec, that dude wasn't Asian. Correct me if I'm wrong but wasn't Mandarin a descendant of Genghis Kahn? I remember he used to to talk about him a lot.

Why is it so hard to make a sequel now? Isn't Ruffalo signed on for 3 films? Make a Hulk 2 with him. I'd like to see an individual Hulk film with him playing the role. I guess Marvel is making him out to be an assemble-only character.
The box office performance was decent for both Hulk films. They made over 240 million each. I don't think you can label either film as as an "out right bomb". I remember reading that The Incredible Hulk sequel will come between Avengers 2 and 3.I think it has more to do with box office performance. We all know Ang Lee's Hulk didn't fare very well (out right bomb) and The Incredible didn't do much better. They are just wary of investing in another solo story.
However, I think most would agree that the Hulk stole the show in Avengers. To me that would warrant another film.

Honestly, I would think a sequel (with Ruffalo) would AT LEAST hit $400m box office. All of the other Avenger films have. And knowing that there's an Avengers 2 coming in 2015, people will want to see the progressing stories of each character. I know I'd be there to see it. But I guess the Hulk is kinda hard to make a solo film out of. Both of the Hulk movies had a lot of boring/slow moments. Couldn't stand the Bana one and Norton was the only reason why I liked the reboot. The Incredible Hulk is my least favorite of the individual Avengers' films.
