An article in the Tennessean says that it's Smith's understanding that the holdup is the validation of his SAT score. If that's true, it would seem that the issue about his high schools has already been cleared, which would seem to be good news. I mean, how could they invalidate a test score without a showing that he cheated? Tennessean Article

It depends on how much his score improved. They flag any test that shows an improvement above a certain threshold. It is in their discretion to make the student athlete take the standardized test yet again.

So they presume foul play and require the student athlete to overcome the presumption by taking the test again?

Exactly. If a score improves by more than whatever they have designated an acceptable amount, they can simply act as if it never happened.

It would be considerate of the NCAA to make a decision that would allow Smith to begin his studies when classes start this month.

fingers crossed, it seems to me that the NCAA should be focused on trying to let guys get an education (with all the resources an athletic dept can provide) than finding reasons and ways to keep them out

A reasonable thought. However, reason and the NCAA are as compatable as Axel Rose and Vince Neil.
