If they both unfollowed one another and then started following each other again, they would both be at the top of one another's list because the most recent follows are at the top.

Freeze apparently blocked Richmond last night. He unblocked him tonight and that's why they are back in the same place as they were last night before all hell broke loose.

Not necessarily. If you unfollow someone then follow them back after a short period of time, they will show up in the same spot on your "following" list as they were previously.
