Everybody who reads To' oto' o can see the accent marks on the first and third syllables. How hard is that to understand?

I didn't understand it, until I read in Wikipedia that the okina (approximated by the apostrophe) represents a glottal stop, rather than an accent.

I don't think those are intended as accent marks. I think in this case it is more analogous to apostrophes.
 
The apostrophes are intended as glottal stops and each is called a koma liliu. His last name's proper pronunciation is here:
