if I have that feature in my car where I can open the garage door by pushing a button on my rear view mirror, does the actual remote have to be in the car always?
If you are talking about the Homelink system, once you program the buttons in the Homelink system, you do not need the remote that came with the garage opener.
 
Is it difficult to program?
We have three vehicles that have Homelink. It's very easy to program. You push a button on the opener and then on the Homelink. It reads the signal and boom you're done.

FYI, I added a Homelink to my Jeep. I bought it off of eBay from a Mercedies SLK. I installed it so that it powers off of my rearview mirror. It doesn't work when the key is off. It makes it safer, since I take the top off often.
