It's possible, but the selection on runners is insane and makes it unlikely that someone would make a fatal mistake.
I believe it will eventually happen, not that I'm criticizing it. Some people just have an insane drive that pushes them to dangerous points to succeed. Like people who get so wiped out while climbing Everest they have no strength to come back down.
Yes, if it goes on long enough it may happen. Unlike Everest, though, you can't just buy your way into Barkley. The competitors are proven ultramarathoners and experienced orienteers. If someone dies, I believe it would be from either a freak accident or something like an unknown health issue.
I'm actually quite surprised that no one has died yet -- either from rhabdomyolysis, CV event, or by accident/fall in the dark. Make no mistake: this is not your typical ultra-long run.
I think the extreme attrition of just loop one has something to do with that. People are almost immediately culled and miss the cutoff. Also, isn't rhabdo more prominent in athletes that aren't used to the abuse? These competitors have all been doing ultras for years and their body has become used to it.
You have a good point: a good chunk of the participants throw in the towel early. Those that continue on little to no sleep with questionable nutrition and hydration are still really testing the limits of the body. I believe I read that a complete run requires as much hill-climbing as summiting Everest.
