The dossier would never have made it through the scrutiny of the normal channels - it was used despite all of the red flags - some (Comey, McCabe, et al) in the FBI knew it was likely garbage, but used it to get the FISA spying warrants anyway.

The emerging information from CBS News seems to clearly point out they either a) knew it was obviously fake from the beginning and chose to press on or b) are grossly incompetent. Both are firing offenses one comes with possibly criminal repercussions
