I umpired high school, and minor college, baseball for years, and I never understood why base runners even tried to advance after missing a bag. Players are coached to watch carefully; it's almost impossible to get away with it.

Many teams have a coach (or player on the bench) whose sole job is to watch for that. The runners know they missed it. Just go back and live to fight another day.

exactly; I had to call a kid out after he hit a grand slam and didn't touch home plate. Catcher appealed; no one knew what the delay in my call was because I had to wait until he returned to the entrance of the dugout to declare him out since it happened during a dead ball.
