Maybe a bit too philosophical but charity is at the heart of our nation. Self-reliance is a founding principle but that's matched by localized charity. Shifting this to the federal government erodes both ethos. (ethoses?)

That and no one will have an honest discussion about what is the absolute minimum level of HC that people should expect.

I surely don't support fertility treatment for people getting government HC. Hardly a medical necessity but we can't have an honest talk about medical necessity.
