Just how many people out of that 42 million can afford to pay out of pocket for surgery and day to day health care? I would think it would be under 2%.

even if someone has health insurance, expenses can go out of sight quickly. 20% copay on a gazillion is a lot of money to fork over.

access to health care is not the core issue -- cost is.

i read a book that said anywhere from 15-20 million chose not to get health insurance.
