I would say first pharmaceutical companies, then passed onto hospitals.
That has nothing to do with the services they provide. That's the biggest cost. A hospital is a government agency in it's own right, because there is a monopoly on intrastate healthcare and insurance. If people JUST had the choice to go to anyone they want to, anywhere in the country, I would say that premiums would go down 30% or more in 3-5 years.
