I’m glad Freak stepped in on the healthcare issue. I wanted to post more last night but didn’t want to continue to sabotage the RFFT thread.
So many problems and everyone thinks it’s the other party’s fault in healthcare.
I work for a not for profit. I see an average of 20 patients per day which is way more than the average Hospitalist NP. I make about $40,000 less than the average. I’m salary and would make the same if I saw 5 patients a day.
I absolutely hate the financial side of healthcare. I just want to help people if I can. I could take another job easily. Been offered by folks. I love my job though. My hospital does a nice job of providing compassionate care for those in need. I have patients that we provide weekly service for that never sees a bill. I know them well now. They know me and my family
. I would hate to walk away from those relationships and knowing the impact on their lives.
We absolutely turn a profit still. So to keep not for profit status we have obtained multiple critical access hospitals that were failing. We spread the profit from our hospital to keep their doors open and provide care for smaller communities.
Unfortunately it’s a business. So many negatives. Can’t tell you how many times I’ve sat with a patient that has just been told they have a blood clot in their lungs. They are crying their eyes out. Not because they have a clot but because I have to put them on medication that costs anywhere from 500-800 a month AFTER insurance. I contacted manufacturer of Eliquis and they provided us a “starter kit”. Free first month supply. It’s something but still. Other option is Warfarin. Much much cheaper But you have to follow so close with your PCP for weekly labs. How much does Co pay cost at the end of the month?
I don’t know the answer. Government ran healthcare has been a disaster in some areas the world. Do you want the Government telling you your torn ACL is an elective procedure? Or you can’t have this life saving procedure because you’re too old or too fat and statistically will die soon anyway? This happens.
Like it or not things changed for the worse when we tied Medicare reimbursement to patient satisfaction. We live in WebMD age where everyone thinks they know what’s wrong with them and how to care for their illness. Can’t tell you how many times I have walked into a room and said your nuclear medicine stress was negative for ischemia and they are mad!!!??? I’m like it’s a good thing. The patient then demands an arteriogram which a procedure that runs a line into your heart to look for blockages. Inevitably they are pissed. Take the survey by Medicare and hospital gets a reduced amount paid for service because they wanted the procedure.
Ultimately everyone suffers. I just want to help
